Media Sharing:

In 2012, the "Life Book for individuals with intellectual disabilities " project was piloted. The content of Life Book would be reviewed and edited by professional social workers annually through interviews and/or home visits.

The book includes personal information in different aspects of individuals with intellectual disabilities.

The content is summarized into the following categories:

  • Background information – including personal and family background and current situation, services received, etc.
  • Characteristics - including temperament, patterns of behavior, preferences, etc.
  • Living habits - including daily life, diet, social interaction, hobbies, etc.
  • Ability assessment -- including communication, self-care, vocational abilities, etc.
  • Study and vocational training records
  • Health and medical records
  • Personal expectations for the future

These records allow more efficient transactions between different caregivers and ensure service targets maintain their quality of life. On the other hand, parents, as temporarily case managers, can plan for the future with their children based on Life Journey Book with assistance of our social workers.

The "Life Book for People with Intellectual Disabilities" functions as a case manager as it aims to provide comprehensive and systematic support for people with intellectual disabilities at each stage. Before legislation of case manager in social service, increasing fundings for social services to compensate the current shortcomings is recommended.
